‘Syndesmotic Injury’. Ms D is a keen rugby player and during a game with her team an opponent fell on her ankle causing it to twist badly. She was unable to finish the game and limped out of the field assisted by her teammates as walking on it was very painful. After attending A&E an X-ray was performed and fractures were ruled out. She was informed to use a boot, walk it off and that a virtual fracture clinic appointment would be scheduled. After this telephone appointment a member of the hospital confirmed the diagnosis of an ankle sprain and no further appointments or treatment were given.

She came to my clinic 6 weeks after the injury and still unable to weight bear comfortably, not having returned to rugby training or running. On clinical examination the ankle appeared still very swollen and tender with tests suggesting more severe ligament damage.

An MRI confirmed the suspicion of a high ankle sprain grade 3 or syndesmotic disruption. As such this injury requires urgent surgery to avoid developing arthritis in the ankle joint. On the same week, she underwent an arthroscopic surgery to clean up the joint from debris as well as a minimally-invasive fixation of the ligaments. She successfully recovered and made a full recovery returning to rugby painfree.

‘Arthroscopic ankle ligament repair’. ‘Despite other treatments, injuries kept happening. I decided that surgery would be a more permanent solution for me. What I mostly appreciated is that the recovery was already set up for me, including physio, podiatrist, boot fitting, etc.

‘Non-operative solution’. ‘During a game of 5 a side football I sustained an ankle injury that I never had treated properly and would hurt when doing triathlon. Six months later as I continued with pain I consulted with Mr Malagelada who performed a thorough examination and with an MRI scan diagnosed a ligament sprain that hadn’t healed. I didn’t want surgery and he proposed a treatmend based on physiotherapy and an injection that got me back on track with triathlons painfree.
